CEERISK Consulting was instructed by operators of a Cat 980K loader to investigate the origin and cause of the fire in the loader, which was operated in one of UAE’s industrial plants. During our investigation we inspected the subject machine and closely examined artefacts while document the burn patterns. We interviewed eyewitnesses and closely examined repair and service records. We arranged to examine an exemplar at a different location while analysing the failure modes.
Based on our investigation, we were able to determine the root cause of the fire which involved a components failure. A full report was issued to the client followed by a detailed presentation to a team of the client’s engineers and managers. CEERISK’s findings were used to implement corrective action to prevent re-occurrence of the fire loss.
